Fixes

• If a template had duplicate inputs (based on an issue in the builder where copy-pasting inputs could retain the same unique identifier for the input), the submission page for 3B Docs could fail to execute correctly. We have added a patch to resolve this and update all inputs to have unique IDs. No post-upgrade actions are required.
